Embed a Widget
A widget puts your NPS survey straight onto your own website or app, so customers can give a score without going to a separate page. Each form can have several widgets, each with its own embed snippet and settings.
Step 1: Create a widget
- Go to NPS > Forms and open your form.
- Go to the Widgets tab.
- Click Create widget and give it a name (for example, "In-app footer").
The widget opens on its own page with a settings form, a live preview, an embed snippet, and a Draft/Published toggle.
Step 2: Set the widget up

- Score question: override the wording of the score question for this widget only. Leave it blank to use the form's own wording.
Allowed domains
One domain per line. The widget only loads, and only accepts responses, on the domains you list here. Use *.example.com to match all subdomains.
If you leave this blank, the widget will load and accept responses on any website. Add at least one domain before you publish.

A domain is filled in for you from your project's website when you create the widget.
Style
- Background colour
- Text colour
- Accent colour, used for buttons and the selected score
- Font
The preview updates as you type.
Step 3: Publish the widget
Switch the Draft/Published toggle to Published.
The widget also needs its form to be published. If the form is still a Draft, the widget will not load on your site, and a warning is shown on the widget page. Publish the form on its Edit form tab.

Step 4: Add the snippet to your site
While the widget is Published, the Embed code card shows a snippet like this:
Paste it into your site's HTML wherever you want the widget to appear.
Swap {{customer_email}} and {{customer_name}} for the signed-in user's real email and name, using your own site's templating. This ties each response to the right customer. If you cannot pass an email, the widget asks the customer for one before it submits.
Check it is installed
The embed card shows an install status. Once the widget has been loaded on your site a few times, it changes to Installed and receiving views. Until then it shows Not installed yet.
The widget reports anonymous view and response counts only. No visitor data or cookies are collected.
