Customise a Form

After you create a form , you can change its look, its questions, and add a reward for customers who complete it.

Open the form builder

1. Go to Testimonials > Forms.
2. Click your form.
3. Go to the Edit form tab.

You will see:

• Page thumbnails along the bottom. Each thumbnail is one page of your form.
• A live preview of the current page in the middle.
• A Design panel on the right.

Use Preview at any time to see the form as a customer would see it. Click Save changes to save your work.

Edit the fields on a page

Click a page thumbnail to open it. You can add, remove, and reorder fields on that page. The following field types are available:

• Short text
• Long text
• Rating
• NPS score
• Email
• Consent
• Number
• Radio (pick one option)
• Select (dropdown)
• Multi select
• Image upload
• Video upload: customers can record a video straight from their webcam, or upload a video file

The pages themselves are fixed by the template you picked when you created the form. You can turn a page on or off, but you cannot add a brand new page type.

Change the design

In the Design panel, go to the Design tab:

• Logo: upload your logo.
• Background color
• Heading font and heading color, pick from a list of Google Fonts.
• Body font and body color
• Button (accent) color

Add a reward for completing the form

In the Design panel, go to the Reward tab:

1. Turn on Show a reward.
2. Write a Message, for example: "Thanks for your feedback! Here's 20% off your next order."
3. Optionally, add a Code, for example: "THANKYOU20".

The message and code appear on the thank-you screen after a customer submits the form.

The same code is shown to everyone who completes the form. It is not a unique code per person.

Publish your changes

Changes you save do not go live until you switch the form to Published using the toggle at the top of the Edit form tab.
