Customise a Form
After you create a form , you can change its questions and its look in the form builder.
Open the form builder
- Go to NPS > Forms.
- Click your form.
- Go to the Edit form tab.
You will see:
- Page thumbnails along the bottom. Each thumbnail is one page of your form.
- A live preview of the current page in the middle.
- A Design panel on the right.
Use Preview at any time to see the form as a customer would see it. Click Save changes to save your work.
Edit the fields on a page
Click a page thumbnail to open it. You can add, remove, and reorder fields on that page. The available field types are:
- Short text
- Long text
- Rating
- NPS score
- Consent
- Number
- Radio (pick one option)
- Select (dropdown)
- Multi select
- Image upload
- Video upload
Keep one NPS score field and one Email field on the form at all times. Without both, customers cannot submit a score.

A common setup is a single page with the score question, an email field, and an optional comment box. You can also add more pages, for example a short "What is the main reason for your score?" follow-up.
Change the design
In the Design panel, open the Design tab:
- Logo: upload your logo.
- Background colour
- Heading font and heading colour, pick from a list of Google Fonts.
- Body font and body colour
- Button (accent) colour
Publish your changes
Changes you save do not go live until the form is Published. Use the Draft/Published toggle at the top of the Edit form tab.
