Create a Form
A testimonial form is the questionnaire your customers fill in to leave a review. This guide shows you how to create one and make it live.
Step 1: Open the Forms page
- Go to Testimonials > Forms.
- Click New form.
Step 2: Choose a template
The Create testimonial form dialog shows two templates. Pages are fixed once you create a form, but you can turn individual pages on or off later.
- Standard (with feedback branching): Best for most businesses. The form first asks for a star rating.If the customer gives 3 stars or more, they go on to the full review flow: a review question, their name/role/photo, their company name/logo/industry/size, a consent question, and a summary page.If the customer gives fewer than 3 stars, they are sent to a private question instead, asking what you could do better. This answer is never shown in public.
- Quick testimonial: A single straight-through flow: an intro, one long-text review question, and a thank-you page. Use this when you want the shortest possible form.
Step 3: Name your form and create it
Give the form a name (for example, "Post-purchase review") and click Create.
Step 4: Publish the form
Open the form and go to the Edit form tab. When you are happy with it, switch the Draft/Published toggle at the top to Published.
Your form's public link only works once the form is Published. If a form is still a Draft, customers cannot submit reviews through it.

Step 5: Get your share link
Go to the Overview tab. Once your form is Published, you will see a Share link with a copy button. This is the public web address you send to customers.
