Create a Form
An NPS form is the short survey your customers fill in to give you a score. This guide shows you how to create one and make it live.
Step 1: Open the Forms page
- Go to NPS > Forms.
- Click New form.
Step 2: Name your form and create it
In the Create NPS form dialog, give the form a name (for example, "Quarterly NPS survey") and click Create.
A new form starts as a Draft with no questions on it yet. You add those in the next step.
Step 3: Add your questions
- Open the form and go to the Edit form tab.
- Add these two fields at a minimum:An NPS score field. This is the 0 to 10 scale.An Email field. This is how each response is tied to the right customer.
- Optionally add a Long text field for a follow-up comment, and a Short text field for the customer's name.
A form cannot take responses without both an NPS score field and an email field. If either is missing, customers see an error when they try to submit.

Step 4: Publish the form
On the Edit form tab, switch the Draft/Published toggle at the top to Published.
Your form's public link only works once the form is Published. While it is a Draft, customers cannot submit a score through it.

Step 5: Get your share link
Go to the Overview tab. Once the form is Published, you will see a Share link with a copy button. This is the public web address you send to customers.
